Background Check and Screening

Prior to registering for some of the courses within Multi-Skilled Health you will be required to complete a Background Check and Drug Screen at the expense of the student. Below are the forms for the current programs that require this process:

• EKG Fingerprint and Drug Test Package
• Pharmacy Tech BI/BCI Background Check Form
• Pharmacy Tech Drug Screen Form
• Phlebotomy Fingerprint and Drug Test Package

Health Record

In addition, students will be required to complete a health record for Phlebotomy and EKG. You can visit the Health Records page and download the health record specific the program you are pursuing.
