Financial Aid changes take effect July 6 for estimated aid and some grants. Here's what you need to do.
Columbus State will be closed July 3 to honor Independence Day, with limited classes and services on July 4.

 

Forms

Please only complete forms that have been requested by the Financial Aid Office.

In addition to forms, you should check:

  • Your Columbus State student email regularly to make sure you are up to date on any important information

For students attending Summer 2026:

  • Manage your financial aid offer in CougarWeb / Self-Service. You will find important messages and tasks there. See 2025-26 chart section below for forms information.

For students attending Autumn 2026 and later:

  • Manage your financial aid offer in Workday. You will find important messages and tasks there. See 2026-27 chart section below for forms information.

2026-27 Offer Revision Request Instructions

To request an adjustment to your current 2026-27 Financial Aid Offer, complete a 2026-27 Financial Aid Offer Revision Request as directed below.

  • To access the document, log in to your Workday account - cscc.edu/workday
  • Type 'Create Request' in the search bar.    
  • The Create Request box will open. Type 'Revision' in the Request Type.
  • Select the appropriate form (2026-27 Financial Aid Offer Revision Request) and click OK. Read and complete the information as instructed.  NOTE: If you are a Columbus Promise student, make sure to choose that designated form. 
  • Once you have completed the form, hit Submit.

NOTE: This form may also be used to reinstate a Pell grant that was mistakenly declined. 

Once the form has been processed by financial aid, a student note will be added if the request was denied or if the form was sent back to you for adjustment.  If the request was approved, a notification will be available for you to view.

Depending on the volume of forms submitted, typical turnaround time will be 2 to 5 days for updated information to be reflected on your Workday account.

2026-27 Federal Direct Parent PLUS Loan Revision Form

Satisfactory Academic Progress (SAP) Appeal Forms will be available in Workday in late Autumn 2026.
