Campus Event/Meeting Accommodation Request form

 

It is Columbus State policy to provide reasonable accommodations to students with disabilities as stated in the Americans with Disabilities Act (ADA) and Section 504 of the Rehabilitation Act. Please use this form to request access to accommodations at a Columbus State event or meeting. Please note that accommodation requests must be made in advance of the event/your appointment. Some accommodations require two weeks advance notice. Although attempts will be made to honor requests made on short notice, requests which do not allow enough notice increase the possibility that services will be unavailable.
