Student Information Updates

To ensure that Columbus State can effectively communicate with and provide support to students, students are encouraged to keep their personal information up-to-date. Students can update or correct information through the following methods:
- Self-Service - Updates to mailing and email addresses, emergency contacts and proxy information, as well Personal Identity Options (Chosen Name, Preferred Pronouns, or Gender Identity) can be completed by the student in Self-Service. Please log into cougarweb.cscc.edu and navigate to "User Options."
- Student Information Update Form - This form is used to process legal name changes, record a student's Selective Service
Number, or to correct any errors with a student's legal name, date of birth, or Social
Security Number. Please carefully review the form for additional details about the
documentation required for each type of update. The form can be submitted in the following
ways:
- In person: Visit Student Central at our downtown campus or Delaware campus Student Services, with your picture ID and any additional required documentation
- Electronically: Email a completed form to changeinfo@cscc.edu, and include any additional documents as attachments. Please note: Students must use their CSCC student email when submitting this form for security and identification purposes.
- Program of Study Updates - Students can update their primary program of study or a certificate using any of
the options below:
- Meet with an academic advisor*
- Call the Telephone Information Center at 614-287-5353
- Visit Student Central (Columbus Campus) or Student Services (Delaware Campus) in person
- Use the Student Central chat feature
*Important: Approval from an advisor is required if requesting to add a second program of study!
