Wellbeing at Columbus State
Columbus State’s Wellbeing 360 initiative, featuring the Engagement Program and Rewards
Program, will help our full-time employees engage in their health and wellbeing and
support opportunities to be rewarded for investing in their wellbeing.
Wellbeing 360 includes two parts:
- Wellbeing 360 Engagement Program: A streamlined program of health activities, including a wellness assessment and annual biometric screening. Participation in the Wellbeing 360 Engagement Program is required to maintain your 20% medical premium. Jump to the Wellbeing 360 Engagement Program.
- Wellbeing 360 Rewards Program: A new program designed to help employees thrive by supporting wellbeing and overall health. Complete wellness activities to earn up to $300 in gift cards. Jump to Wellbeing 360 Rewards Program.
Wellbeing 360 will also provide employees with a website portal to access wellbeing resources, tools to set personal goals, challenges, and more. Wellbeing 360 is administered by the CSCC Benefits & Wellbeing team, which is dedicated to helping employees feel confident and knowledgeable about what resources are available and how to access programs and education.

Wellbeing 360 Engagement Program
Columbus State’s Wellbeing 360 is a new initiative designed to help employees thrive with programs that support health and wellbeing.
The streamlined Wellbeing 360 Engagement Program, in partnership with OhioHealth Employer Solutions, includes a brief Wellness Assessment and annual Biometric Screening to help insured, full-time employees, their spouses, or domestic partners learn more about their health and wellbeing while maintaining the 20% medical premium.
Complete the Wellness Assessment and Biometric Screening by June 30, 2026, to maintain your 20% medical premium. Those who do not complete the Wellness Assessment and Biometric Screening are responsible for paying 30% of your medical premium.
The Engagement Program does not apply to employees who are members of CSEA or Teamsters.
Wellbeing 360 Wellness Assessment
The Wellness Assessment is a 10- to 15-minute online survey that asks about your overall health, physical activity, wellness habits, health history, and past checkups. It helps identify what health risks you might have and delivers tips, tools, and other resources tailored to your unique wellbeing needs on the OhioHealth wellness portal.
Wellbeing 360 Biometric Screening
Complete your Biometric Screening, which includes blood work, with a primary care physician or at one of Columbus State’s on-site wellbeing events later this year, to learn more about your health numbers and the many ways you can thrive in your health and wellbeing.
Wellbeing 360 Rewards Program
Columbus State’s Wellbeing 360 is a new initiative designed to help employees thrive by supporting wellbeing and overall health.
With our new Wellbeing 360 Rewards Program, all full-time Columbus State employees can earn up to $300 in gift cards annually. Choose from a wide variety of wellness activities important to you—from educational sessions, cooking demonstrations and fitness classes, to emotional wellbeing challenges, or financial and personal growth experiences.
Once complete, you’ll earn your choice of easy-to-use gift cards for more than 150 retailers and restaurants, including Amazon, Nordstrom Rack, Starbucks, StubHub, Chipotle, Topgolf, and many others.

WELLBEING 360 OVERVIEW
Based on feedback about the previous Rally program, Columbus State is moving to a streamlined initiative, Wellbeing 360. Wellbeing 360 will help employees engage in their health and wellbeing and support opportunities to be rewarded for investing in their wellbeing. Wellbeing 360 will also provide employees with a website portal to access wellbeing resources, tools to set personal goals, challenges, and more.
Columbus State is investing in employee wellbeing because employees are an critical part of the institution. Columbus State approaches wellbeing through the eight dimensions defined by SAMHSA (Substance Abuse and Mental Health Services Administration): emotional, financial, social, spiritual, physical, occupational, intellectual, and environmental. Wellbeing 360 will give employees the opportunity to focus on the eight dimensions of wellbeing and get a return on that investment.
We continue to partner with current vendors including AccelWell, Matrix EAP, CancerBridge, and BMI Federal Credit Union. We are also excited to announce our new partnership with OhioHealth Employer Solutions for additional wellbeing offerings, including the new wellness portal. This list is not a comprehensive list of all of Columbus State’s partners.
Yes, we have partnered with OhioHealth Employer Solutions for a new wellness portal, powered by WebMD ONE. This is a full replacement of the Rally portal. See the Wellness Portal section below for additional details.
Rally coins must be earned by June 30, 2025, for the current plan year. They do not carry over to Wellbeing 360.
WELLNESS PORTAL
Click here to access the wellness portal. Employees can also access the portal via the Columbus State employee webpage. The first time you visit the wellness portal, you will complete your registration. During registration, you will create a username and password to use when signing in to the wellness portal. If you access the wellness portal outside of the Columbus State employee webpage, you will need to login with the username and password that you create during registration.
Employees: Your Registration ID is your Cougar ID. This can be found on your employee ID or in Workday in your profile.
Spouses/Domestic Partners: Your registration ID is the first five letters of your last name + first letter of your first name + your date of birth (MMDDYY). For example, if your name is John Anderson and your date of birth is October 15, 1960, you would enter ANDERJ101560.
- If your last name is shorter than five letters, enter your full last name, followed by first initial, followed by your date of birth (MMDDYY).
The wellness portal is your new one-stop shop for all things wellbeing. Below are some of the free features and benefits of participating:
- Goal-setting and tracking

Your privacy is important to us. Columbus State, OhioHealth, and WebMD comply with all federal and state privacy laws, rules, and regulations to ensure your privacy is protected. Your individual data is never shared with your employer. Columbus State will only have access to aggregate data.

WELLBEING 360 ENGAGEMENT PROGRAM
The Wellbeing 360 Engagement Program helps Columbus State's insured, full-time employees and their spouses/domestic partners maintain their 20% medical premium by completing a brief Wellness Assessment and annual Biometric Screening. Without completing the Engagement Program, employees and their spouses/domestic partners will pay 30% of their medical premium.
Columbus State full-time, insured employees and their spouses or domestic partners are eligible. The engagement program does not apply to employees who are members of the CSEA and Teamsters.
Covered employees must complete the online Wellness Assessment and a Biometric Screening by June 30, 2026, to maintain their 20% medical premium.

Yes. If you are not enrolled in the medical plan but complete the online Wellness Assessment and a Biometric Screening by June 30, 2026, you will earn a $25 electronic gift card that is subject to applicable payroll tax reporting.
Yes. If a spouse/domestic partner is enrolled in the medical plan, both the employee and spouse/domestic partner must complete the online Wellness Assessment and a Biometric Screening by the June 30, 2026, deadline to maintain the 20% medical premium. If the employee and/or spouse/domestic partner do not complete both requirements, the medical premium will be 30%.
Employees hired after January 1, 2026, are not required to complete the Wellness Assessment and Biometric Screening to maintain their 20% medical premium. These employees will be required to complete the tasks during the next plan year (July 1, 2026-June 30, 2027).
Any change in premium will begin on the first paycheck of July in the new plan year.
The online Wellness Assessment is a quick and private survey that helps you understand your health and wellbeing better. The OhioHealth Wellness Assessment takes approximately 10 minutes to complete. It asks a series of questions related to your diet, exercise, sleep, medical history, and lifestyle to create a road map toward a healthier life. It is one of the simplest ways to identify the things you are doing well and the areas where your health behaviors could improve. The information gathered is 100% confidential, and individual data is not released to Columbus State.
You need to utilize the wellness portal (on desktop or mobile app) to complete your online Wellness Assessment.
- Log in to your wellness portal account.
- Click on Wellness Assessment on the homepage or in the drop-down menu.
- Answer the questions in each section.
- Be sure to click Finalize when you are done to submit the assessment.
You are encouraged to complete your Wellness Assessment as soon as possible to take full advantage of the programs and resources available to you.
A Biometric Screening measures basic physical characteristics like your height and weight, blood pressure, blood sugar/A1C, and cholesterol levels, as well as several other health indicators. It provides a current snapshot of your overall health and wellness and identifies risk factors for health conditions such as diabetes, heart disease, or hypertension that could develop down the road.
You have two options for completing a Biometric Screening:
ON-SITE AT WORK — Biometric Screening events will be offered on-site at Columbus State during the plan year. No forms are needed, and results are automatically reported on your behalf.
AT YOUR PHYSICIAN’S OFFICE — Schedule a routine visit with a primary care provider (PCP) and take the Biometric Screening Form with you to your appointment. Follow the instructions provided on the form. You are responsible for making sure that the form is submitted by the June 30, 2026, deadline.
You can download the Biometric Screening Form on the wellness portal. On the Home Page or Wellness Hub tab, click Biometric Screening Form to download and print the form.
Once you or your primary care provider's office have completed your Biometric Screening Form, you can submit the completed and signed form to CSCCwellbeing@ohiohealth.com or fax free of charge to 614-533-0285.
Your incentive credit and results will post to the wellness portal within 15 business days of your on-site screening date or submission of your PCP completed Biometric Screening Form. A notification is sent when the results are posted.
The Biometric Screening must be completed and submitted by June 30, 2026.
Although it is only required to complete the online Wellness Assessment once a year to earn the premium reduction, you can complete it on the wellness portal as many times as you’d like. The Biometric Screening can be completed at an on-site Columbus State screening/at a primary care provider's office once per year. The premium reduction incentive can only be earned one time.
WELLBEING 360 REWARDS PROGRAM
The Wellbeing 360 Rewards Program offers employees the opportunity to earn up to $300 in gift cards every year with the completion of various wellness activities. The program calendar for eligible activities will be updated throughout the year.
All full-time employees.
By completing activities in the rewards program, you are eligible to earn up to $300 in electronic gift cards! Electronic gift card redemption codes will be sent to those who achieve rewards, and you will have your choice from 150+ vendors to redeem a gift card.
Earn 100 points: $25 gift card
Earn 200 points: Additional $50 gift card
Earn 350 points: Additional $75 gift card
Earn 500 points: Additional $150 gift card
Participants are encouraged to complete activities from each of the categories below to earn points in the Rewards Program. Log in to the wellness portal and visit the rewards page for more information on how to complete and earn credit for each activity. The below list of activities will continue to evolve with new opportunities as the College continues to assess the overall program.

The deadline to complete all activities and submit forms/proof of activity is June 30, 2026.
Proof of activity and all applicable forms can be submitted to CSCCwellbeing@ohiohealth.com or faxed free of charge to 614-533-0285. Explore the Rewards Program page on the wellness portal for more forms and detailed directions.
Credit will be reflected on your wellness portal account within 15 days of OhioHealth receiving your form/proof of activity.
Employees will receive electronic gift cards on a quarterly basis. The cards will be distributed by the end of the month following each quarter. Electronic gift cards will be distributed via email through Tango Card. Employees can select the vendor of their choice for gift card redemption.
All gift cards are considered taxable income. Gift card earnings will be taxed on the paycheck following gift card distribution.
There is no penalty to your benefits if you do not participate in the Rewards Program. If you do not complete the first rewards program activity on the wellness portal, which is to consent to have gift cards taxed through payroll, you will be actively opted out of the program. At any time, you can choose to consent to the payroll tax on gift cards and opt in to participate in the Rewards Program.
