Share Feedback: Concerns, Complaints, or Kudos

Columbus State is dedicated to hearing your feedback and resolving concerns in a fair and timely manner.

The forms below can be completed by students, employees or community members to file a report on their own behalf, or by anyone wishing to file a report on behalf of an impacted party.

Please note that submissions using these forms are not reviewed outside of normal business hours, Monday - Friday, 8:00 a.m. to 5:00 p.m. If there is immediate risk to health or safety, contact the Columbus State Police Department at 614-287-2525 or dial 911.

To report a concern, complaint, or compliment, please select the category that best fits the nature of your concern or complaint and follow the instructions.


Discrimination, Sexual Harassment (Title IX) and/or Retaliation

to report information about an alleged incident of discrimination, harrassment, and/or retaliation based on a protected class

Please use the link to the form below if you have information related to an alleged incident(s) of discrimination based on a protected class, sexual harassment (including sexual assault, relationship violence and stalking), and/or retaliation for making a report or participating in an investigation of the same. 

Under College Policy, protected classes are sex, race, color, religion, national origin, ancestry, age, disability, genetic information (GINA), military status, sexual orientation, pregnancy, status as a parent of a young child or a foster parent and gender identity and expression.

Academic Misconduct

for faculty/staff to report alleged violations of academic integrity

Please use this form to report an alleged violation(s) of academic integrity as defined by The Student Code of Conduct, predominantly Cheating and Plagiarism. To report non-academic violations of the Columbus State Student Code of Conduct, please complete the Incident Report form.

Incident Report

to report non-academic behavioral concerns to the Office of Student Conduct, concerning behavior or requests for support to the Behavioral Intervention Team, or a hazing incident to the Columbus State Police Department.

This is a multi-purpose form that can be used to report:
  1. Non-academic behavioral concerns with the Office of Student Conduct (including, but not limited to, disruption of classroom or College activities)
  2. Concerning behaviors or requests for support with the Behavioral Intervention Team
  3. Incidents of hazing with the Columbus State Police Department

    Please Note: matters for the Columbus State Police Department that are not related to hazing should be reported directly to their office by calling 614-287-2525 or 911 for emergencies.

After a form has been submitted, the reporting party will be contacted, when possible, to discuss next steps which could include formal outreach and/or formal charges under the Student Code of Conduct. Hospitalized Student Report

to support students facing medical/mental health hospitalization

Please use this form if a student is impacted by a medical or mental health hospitalization that affects their successful progress at Columbus State. This form can be completed by students on their own behalf, or by a faculty or staff member who has been notified of a student hospitalization.

Please note: Completing this form will allow staff to reach out to the student to discuss academic and financial aid implications to help students make informed decisions about progressing in their courses, withdrawing, taking an incomplete, etc. This information will also be used to connect students with appropriate resources at Columbus State, such as Counseling Services, Accessibility & Testing, etc.

General College Concerns or Feedback

to make suggestions or submit a complaint that does not fit into the above categories

Please use this form to share general feedback or concerns about your experience at CSCC.

Complaints that cannot be resolved by working with the relevant office or department can be submitted using this form. Completing this form ensures that the official College complaint/concern process will be followed and that you will be notified of the outcome. While we can not guarantee a particular outcome, the information you provide gives the College the opportunity to improve our processes and procedures where appropriate.

If your concerns are about an academic course or instructor, please follow the steps below:

  • The first step is to review your syllabus, which explains expectations and requirements of the course, and communicate your question(s) or concern(s) with your instructor
  • If you have completed the above steps and your issues have not been resolved, contact the chair of the department listed in the College directory.
  • If you have completed all of the steps above and your situation is not resolved, your final step is to contact the dean over the division. If you are contacting a dean, it is best to send an email and provide a brief description of your situation with documentation that you have completed the above steps first.
