Request a Certificate
To request a certificate, please follow the steps below:
Submit the Certificate Request
Complete and return the certificate request form to the Information Systems Technology
Department before the end of the semester in which you plan to complete the certificate(s).

Important Notes:
- Certificate applications will be processed after the semester you indicate as your
completion semester.
- You will receive updates regarding your request status via your Columbus State student
email.
- Please ensure that your mailing address is accurate to avoid delays in receiving your
certificate. If you prefer a digital copy, please let us know.
