Health Records
Many health programs require you to fill out a health record before you can enroll in your program of study. These health records may include health history, tuberculosis testing, immunizations, and information of that nature.
The Health Records Office collects and manages health records forms listed under Health Record forms by Program.

Questions or concerns may be addressed by calling 614-287-2450 between the hours of 8:30am-4:00pm. Monday through Friday.
If we are unable to answer your call please leave a voice message, calls will be returned within 1-2 business days.
You may choose to visit our office for in person walk-ins
Mondays or Tuesdays between 10am-3pm.
Phone Appointments are available Wednesdays or Thursdays between 10am to 2:40pm.

Health Record Submission Deadlines
* You may submit your complete record after the deadline; however, this may delay your ability to register for classes by 10 business days.
Completed health records received by the stated deadline, may be processed, and given clearance to register within 10 business days.
Completed health records received after the stated deadline, may take up to 10 business days to process, and given clearance to register. Business days do not include holidays or weekends.
If you have early registration you must submit your complete health records 10 business days prior to the deadlines listed below.

If you are unable to receive Tuberculosis (TB) Skin Testing, one of the blood tests listed below may be used as an alternative:
