Updated Academic Misconduct Reporting form

In an effort to better serve the Columbus State community, the Office of Student Conduct would like to announce an updated Academic Misconduct Reporting form.

The new form, which can be found on Columbus State's new Feedback Page, will guide users through questions specific to matters of academic integrity and allow for better service to our reporting faculty and staff members as well as for our students navigating the Student Conduct process. The new Academic Misconduct Reporting form does not replace our general Incident Reporting form, which should still be used for documenting other concerns that may be violations of the Student Code of Conduct (such as disruptive behavior, etc.), as well as to report concerning matters to the Behavioral Intervention Team. In addition to the link being available on the Campus’ reporting page, the form can be found on the Student Conduct website or by clicking the link below.
