You can download a nomination form here (.pdf) or pick one up at the following locations starting Jan. 19:

The award ceremony will take place at 12:30 p.m. March 3 in the Columbus State Conference Center ballroom. This year’s theme is “The Mind, Body and Spirit of Healthy Leadership,” and the keynote speaker will be Dr. Teresa Long, commissioner of the Columbus Health Department.

Long started with the board in 1986, serving as medical director and assistant health commissioner, before taking the top job in 2002. Before coming to Columbus, she served on the front lines of the emerging AIDS epidemic as a physician specialist with the San Francisco Department of Public Health. More recently, she spearheaded Columbus’ response to the H1N1 virus.

Previous winners have been community volunteers, mothers, entrepreneurs and scholars. They have overcome obstacles such as injury, abuse—or just excelled despite their busy lives.
