Request to Decline Transfer Credit for Loan Forgiveness Purposes
As some for-profit institutions of higher education close their operations, Columbus State Community College is ready to offer a new start to students affected by their closing.
Students who come to Columbus State from one of the closed for-profit institutions to obtain or complete their degree may use The Request to Decline Transfer Credit form in compliance with the loan forgiveness requirements established by the Ohio Department of Higher Education.
Students who had previously attended ITT Technical Institute may use the form for loan forgiveness purposes.
Students who submit official transcripts from other public or private institutions such as The Ohio State University, Franklin University, Capital University may not use this form for loan forgiveness or any other purpose.
