High School Graduation and GED Self Reporting

The High School Graduation/GED Self Report form is to be used in place of the student sending a high school transcript or GED scores to Columbus State. This form may be used to fulfill some requirements for a high school transcript to be submitted, however, some programs do require the transcript for a record of certain courses taken in high school. Check with your academic advisor to find out if a transcript will be required for your program of study. For questions regarding this form you may contact:
