Enrollment and/or Graduation Verification
The
Enrollment Verification form is used to verify the enrollment status
and/or graduation status of the student. Verification will only be
completed for previous quarters or, if currently enrolled, when all fees
have been paid. Should the student has another form, such as an
insurance form, to be completed, the second form must be attached to the
Enrollment Verification form and submitted to the Records and
Registration Department, Madison Hall Room 201 Monday through Thursday
8:00 a.m. to 7:30 p.m., Friday 9:30 a.m. to 4:30 p.m. and Saturday 9:00
a.m. to 12:00 p.m. (noon).
If you need a duplicate copy of your
Columbus State Community College Acceptance Letter, please contact the
Admissions Department at 614-287-2669.
