Early Grade release for Graduates of Other Institutions
The
Early Grade Release for Graduates of Other Institutions form is used
by graduates of other colleges to submit the grades of the course(s) he
or she has taken at Columbus State during the quarter in which the
student will graduate. The student's graduation status is verified at
the other institution before the letter and transcript are sent. This
form is used only for graduates and will not be processed for student's
seeking entry to another institution or to a program at another
institution.
The process is to send a letter via fax to the other institution. The
letter will state the students course(s), grades, and credit hours as
well as the date the official transcript will be mailed. The student is
not required to order and pay for a transcript as that is part of this
process.
The form must be completed with the student's name, CougarID or Tax ID
number (SSN), institution from which he or she is graduating, the name
of the graduation coordinator, the coordinator's telephone number and
fax number along with the student's signature. The form will not be
processed if the information, including the student's signature, is not
complete on the form.
