Request to Withhold Personal Information from Directory
Columbus State Community College, in accordance with the FERPA Act of
1974, has designated the following categories of information about
students as public information:
- Name
- Address(es) (home)
- Telephone (home)
- Program/Technology
- Participation in officially recognized
activities and sports
- Weight and height of members of athletic
teams
- Inclusive dates of enrollment
- Degrees and awards received (to include
Dean's List and Academic Honors List)
- Most recent previous educational agency or
institution attended
You may have this information withheld by completing the
Request to Withhold Personal Information From Directory form. This form is to be presented,
by the student, with a photo ID to the Registration Windows, Madison
Hall, Room 201. If you have any questions regarding the Request to
Withhold Personal information from Directory process, you may contact:
