Dean and Transfer Recommendation Forms to Other Colleges/Universities

 The Dean Recommendation is a form from another college or university requesting approval for a student to attend Columbus State. This form may need to be completed by Columbus State or a letter may need to be mailed by Columbus State to the other college/university. If you have a form or request for a letter, you may contact:
