Prerequisite/Over 22 Credit Hours Permission Form
The Prerequisite/Over 22 Credit Hours Permission Form is to be used by
first- and second-quarter students who have not had official transcripts from other institutions
evaluated by and transfer credit placed on their record at Columbus
State Community College or students who would like to take more than 22
credit hours of courses at Columbus State Community College.
Students must make an appointment to see an advisor in Advising Services, Aquinas Hall, Room 116 to have an unofficial copy
of their transcript from the other institution assessed for the course
that meets the prerequisite or to gain permission to take more than 22
credit hours of courses at Columbus State Community College. You may
contact Advising and Counseling services via telephone at 614-287-2668
to make an appointment.
It is the student's responsibility to have an official transcript from
all other institutions that he or she has attended sent to:
