4.  Fax (614/287-6045), drop-off or mail ONE copy to Ms. Washington or Mr. Bumgarner immediately to have your restrictions lifted.

Note: Your restrictions will be lifted within 24 hours/one business day after the form is received in the K-12 Initiatives Office at Columbus State Community College.  After restriction is lifted, be advised it is YOUR responsibility to change/drop your class(es) and you only have clearance to register ONCE.  After that restrictions are reinstated.
