Need to drop a class or change a class section:
1.  Inform your high school guidance counselor and/or your parents/guardians of the change as this could affect your high school eligibility.
2.  Complete this form
3.  Sign form
4.  Fax (614/287-6045), drop-off or mail ONE copy to Ms. Washington or Mr. Bumgarner immediately to have your restrictions lifted.

5. You may drop the class or change the class section yourself within 24 hours using one of the following:
(a) www.cscc.edu, CougarWeb for Students  (b) Call (614) 287-2287 (c) SITE Terminals (d) Call (614) 287-5353 (e) Go to Records & Registration with the College's Add/Drop form

I have notified my high school guidance counselor and/or my parents/guardians that I will be dropping class or changing the section of the class(es) listed above. I understand it is my responsibility to drop the class or change the class section.  If I am a PSEO student, I must return the book(s) for the class(es) I am dropping.

If you are dropping, you must provide a statement in the space below as to why you are dropping the class.

Note: Your restrictions will be lifted within 24 hours/one business day after the form is received in the K-12 Initiatives Office at Columbus State Community College.  After restriction is lifted, be advised it is YOUR responsibility to change/drop your class(es) and you only have clearance to register ONCE.  After that restrictions are reinstated.
