Forms and Sign-up

To participate in the Intercollegiate Athletics submit an Athletic Information Form, an Athletic Waiver Form, and a Player Profile Form.  Return the forms to the Student Activities/Athletics office located in Delaware Hall, Room 134, (614) 287-5092.

Sign-up is required to participate in intramural sports and wellness programs. Sign-up activities are located in Delaware Hall, Room 134.
